    The East Bay may be a medical Mecca, but finding a good doctor isn’t always easy. In an effort to help patients make informed medical decisions, Best Doctors (www.bestdoctors.com) regularly puts together a national list of physicians who have been deemed the best in their specialty area by their peers.
    Founded in 1989 by Harvard Medical School–affiliated physicians, Best Doctors says its goal is to give consumers information about specific diagnoses, the latest medical advances and where to receive state-of-the-art care. Every two years, Best Doctors polls doctors, asking, “If you or a loved one needed a doctor in your specialty, to whom would you refer them?” Only physicians identified by their peers in previous years as “best doctors” receive the Best Doctors survey.
    Out of 180 physicians named as the best doctors in the East Bay, we chose five outstanding physicians to profile. Each is a recognized leader in his field.
    Spanning a wide variety of specialties from allergy and immunology to child neurology and geriatrics, this list of local physicians serves as a guide for patients seeking some of the best medical specialists the area has to offer.

Dr. Hitendra Patel

Pediatric Cardiology

     Growing up in Kenya, Patel went on to graduate from St Mary’s Hospital Medical School in London, England, and completed pediatric residencies at Dudley Road Hospital, the Hospital for Sick Children and Leeds General Infirmary, all in England. After moving to the United States, he completed a cardiology fellowship at Children’s Hospital of Philadelphia and became board certified in pediatric cardiology. At Children’s Hospital and Research Center Oakland, Patel specializes in cardiac intensive care, and cardiac and interventional catheterization in both pediatric and adult patients with congenital heart disease. Patel, 46, was married last year and lives with his wife in Berkeley.

How has the field of pediatric cardiology changed?
    Over the past 26 years, the field of interventional cardiac catheterization has grown both in scope and complexity for infants and children who have been diagnosed with congenital heart disease. At most hospitals, including Children’s, interventional catheterization procedures outnumber purely diagnostic catheterizations. The main benefit of interventional catheterization is that it alleviates the need for cardiac surgery in many cases. Most patients go home the same day without the need for hospitalization.

Have your patient demographics also changed since you first started practicing medicine?
    The prevalence of obesity has increased drastically in recent years among children ages 2 to 19. It’s not unusual for me to see an 8-year-old who weighs over 200 pounds and suffers from high blood pressure. Childhood obesity has become a huge concern and can lead to health problems including heart disease, type 2 diabetes and asthma. At Children’s, we are in the process of launching a preventative cardiology program. By utilizing our nutritionists, sports medicine staff, pediatricians and other physician specialists, we can encourage many of our young patients to adopt healthier eating habits and a regular exercise regimen.

What is your biggest challenge as a pediatric cardiologist?
    All parents expect their child to be born healthy, and it can be hard to learn your baby has a heart problem, sometimes a life-threatening condition, and needs to have surgery, or in some cases, multiple surgeries. In addition to ensuring that each child receives the best possible medical care, it’s my job to answer a family’s questions and make sure they understand the answers and the course of treatment.

What inspired you to become a doctor?
    I grew up in a small town in Kenya, and I remember three community physicians who were also friends of my family. They conducted house calls and their commitment to medicine inspired me to pursue my medical degree. The majority of my patients are 12 months old or younger, and it’s rewarding to practice at Children’s, where we provide care to all children regardless of their ability to pay.

Dr. David Walton

Obstetrics and Gynecology
Kaiser Permanente Oakland Medical Center
    After finishing medical school at Boston University, Walton completed his residency training in obstetrics and gynecology at the University of Hawaii. He then voluntarily enlisted in the United States Air Force and worked as a staff obstetrician and gynecologist at a small U.S. Air Force base in Northern Japan. After completing his three-year assignment, Walton and his family moved to Florida where he entered a maternal-fetal fellowship at the University of Miami. Upon completing the fellowship, he joined Kaiser Permanente in 1992. For the past 20 years, Walton has been an active member of the California Air National Guard. He also serves as chief of perinatology for Kaiser Permanente Northern California. Walton, 53, lives in San Francisco with his wife and is the father of two daughters, 23 and 21.

How has your patient population changed over the years?
    We’re probably seeing twice as many older moms, and we’re also seeing more pregnant women who suffer from diabetes and hypertension. Many of our patients are very health-conscious, but we also see patients who suffer from obesity. This means bigger babies, making the delivery harder, and often leading to a C-section.

Has any new technology been introduced recently in the field of obstetrics and gynecology?
    Yes. Years ago, if a pregnant woman over 35 opted to have prenatal genetic testing (to determine if her fetus had chromosomal abnormalities, such as Down syndrome), she would have to undergo an amniocentesis. For the past year and a half, we’ve been using new technology that allows us to perform a high-resolution ultrasound, known as first-trimester screening, that measures nuchal translucency, a transparent space behind the fetus’ neck that’s temporarily visible in the first trimester. A widening of this space is linked to an increased risk of chromosomal abnormalities.

Tell us about your work with the California Air National Guard.
    I train with the National Guard one weekend each month and go on deployments throughout the year. We were in Guatemala for two weeks in August on a medical mission where we saw numerous patients who lived in rural villages and had little or no access to medical care. We had a small, portable ultrasound machine and were able to take photos of fetuses for expectant moms, conduct exams and distribute items such as prenatal vitamins and Tylenol.
    It was an incredible experience. My primary role in the National Guard is serving as a physician to our flight teams.

If you could offer your patients one piece of health advice, what would it be?
    Start taking folic acid before you get pregnant. All women, especially younger women ages 18 to 24, need 400 micrograms of folic acid daily through fortified foods, supplements or both. In pregnant women, consuming folic acid can help prevent serious birth defects. My other piece of advice would be to do your best to attain a healthy lifestyle and get regular exercise. I take my own advice by biking to work two to three days a week.

Dr. Ted Tsukayama

Internal Medicine

   After graduating from the University of California Berkeley, Tsukayama attended the John A. Burns School of Medicine at the University of Hawaii in Honolulu. He started his internal medicine residency in 1982 at Kaiser’s Oakland Medical Center and never left. He serves as an assistant clinical professor of medicine at the University of California San Francisco and lives in Oakland with his wife of 20 years and his three children ages 17, 15 and 12.

What do you enjoy the most about your practice?
    In internal medicine, you treat a variety of patients with different conditions, so every day is a different challenge. I enjoy helping my patients to make the right healthcare decisions to achieve the best possible outcomes. Since I live in Oakland, I often run into my patients around town at stores and restaurants. Even though Oakland is a big city, I often feel like a small-town doctor, and I enjoy that sense of community.

If you were to offer your patients one piece of health advice, what would it be?
    I tell my patients to develop a passion in their life. It’s hard to tell people to get out there and exercise, but if they have a passion like hiking or biking, then exercise doesn’t seem like a chore. I also tell people not to overeat. Many people have a problem adhering to specific diets, so I encourage my patients to eat the foods they like, just in moderation.

What has been your proudest accomplishment as a physician?
    I’ve been able to help a lot of patients and their families after they received a difficult diagnosis. It’s my job to sit down and counsel them about the consequences of different medications and procedures, to find out what their goals are, and help them go down the correct path as they make some hard decisions. There’s no greater compliment than having a family member request you as their doctor because of the difference you made in their loved one’s life.

How do you enjoy spending your time outside of work?
    I recently started biking to work, a distance of three and a half miles each way. I just turned 52 and I jokingly call this my “midlife crisis.” I like to spend as much time as possible with my family and to attend my kids’ soccer and baseball games and concerts. Childhood is so short that I want to enjoy as many special moments as possible in my kids’ lives.

Dr. Mark D’Esposito

    D’Esposito grew up in New York City and earned his medical degree at the SUNY Health Sciences Center at
Syracuse. He completed his clinical training in neurology at the Boston University Medical Center. After residency training, he was awarded a Javits Fellowship to pursue research training at the Memory Disorders Research Center at Boston University and Braintree Rehabilitation Hospital. In 1993, he was appointed assistant professor of neurology at the University of Pennsylvania School of Medicine, where he was also the chief of the cognitive neurology division. In 2000, he was recruited to the Helen Wills Neuroscience Institute and Department of Psychology at the University of California Berkeley to become a professor of neuroscience and psychology, and the inaugural director of the newly created Henry H. Wheeler Jr. Brain Imaging Center. He is also a practicing neurologist at the Veterans Affairs Northern California Health System’s Medical Center in Martinez and an adjunct professor of neurology at the University of California San Francisco.
    In 2007, D’ Esposito was elected chairman of the Organization of Human Brain Mapping and president of the Society for Behavioral and Cognitive Neurology. He lives in El Cerrito with his two children ages 13 and 9.

How has the scope of your practice changed over the years?
    Earlier in my career, my practice was predominantly patients with dementia and Alzheimer’s disease. Since arriving in California I have concentrated on neurorehabilitation, especially with patients who have had a stroke or traumatic brain injury.

What has been your biggest challenge as a neurologist?
    My goal has always been to find effective treatments that help my patients. In my specialty, cognitive neurology, that means either pharmacological treatments or cognitive remediation treatments. I am actively involved in research for both types.

Can you tell us more about the research you’re currently conducting at UC Berkeley?
    My research focuses on investigating how the brain creates mental experiences such as forming memories and making decisions. My lab uses a brain imaging method called functional MRI to identify the brain regions and the course of brain activity. While traditional MRIs take a photo of the brain, they don’t tell us anything about cognitive function. A functional MRI can help us to track a patient’s recovery from brain injury and help us determine what therapies might work best. I also conduct research studies with patients who have suffered memory loss from stroke, traumatic brain injury, Parkinson’s disease and Alzheimer’s disease to further understand the brain mechanisms that underlie memory function. Finally, we are attempting to understand normal aging and why individuals become forgetful as they get older. We ultimately hope our findings will lead to earlier detection of neurologic disorders such as Alzheimer’s and Parkinson’s.

Do you think researchers are close to discovering a cure for neurological conditions such as Parkinson’s?
    Absolutely. I think we are closer to finding cures for many neurological disorders, and I believe I will see cures in my lifetime for both Parkinson’s and Alzheimer’s disease. We are already exploring ways to detect Alzheimer’s earlier with functional MRIs. If we can begin treating patients before they become symptomatic, we could stop the progression of the disease until a cure is found.

Dr. Katie Rodan

Dermatology

    Rodan, 53, was born in Southern California and received her undergraduate degree in history from the University of Virginia, and her medical degree from the University of Southern California School of Medicine in Los Angeles. She completed her internship at Los Angeles County Hospital and her residency in dermatology at Stanford University School of Medicine, where she was appointed chief resident.
    A world-recognized expert in her field, Rodan is the co-developer of the Proactiv Solution, a three-step acne management system, and the Rodan and Fields line of skin-care products developed with her business partner, Dr. Kathy Fields, a San Francisco dermatologist. Rodan has a private practice in medical, surgical and cosmetic dermatology in Oakland. She is the mother of two daughters, 22 and 19, and resides in the Oakland area with her husband.

How has the field of dermatology evolved?
    I used to treat a lot of women and teens suffering from acne. When I moved my office from Fremont to Oakland eight years ago, I began practicing cosmetic dermatology. My clients are women, primarily in their 40s to 60s, who want to look as good on the outside as they feel on the inside. Some of my more popular anti-aging procedures include Botox, a cosmetic injection for frown lines, and Restylane, a cosmetic dermal filler. We also use lasers to treat patients who have redness, wrinkles and broken facial capillaries.

So is 60 really the new 40, and have society’s views on aging changed over the years?
    Absolutely. I look back at what 60 looked like in the media when I was a child, and I remember images of Granny Clampett from The Beverly Hillbillies and Aunt Bee from The Andy Griffith Show. Today’s images of 60 are vital, beautiful women like Goldie Hawn and Susan Sarandon who are still in the game. Both of my parents are in their 80s and continue to work full time. The last thing I want to think about is retirement—my job is my youth pill.

Your Proactiv Solution, which debuted in 1995, has helped a lot of people effectively fight acne. How did the idea for Proactiv come to fruition?
    I suffered from acne as a teenager, and I know how the condition affects a person’s self-esteem and confidence. My colleague, Dr. Kathy Fields, and I saw a huge hole in the way that acne was being treated. Oral antibiotics used to be one of the only ways to prevent acne, but they aren’t the best solution for long-term use. We weren’t looking to create a big business. We just wanted to create a product that would help treat and
prevent acne in people of all ages.

Are you in the process of developing any new skin-care products?
    Dr. Fields and I hope to be to skin care what Dr. Scholl is to the podiatry field. We are developing many new products that effectively treat common dermatologic conditions and are tackling problems such as sun damage with our new Reverse product line. We also recently made our products available through independent consultants as opposed to being sold in retail stores. We’re excited to offer women the opportunity to work from home and be their own boss.
